1. CHANGE ORDER HEADER
This Change Order is entered by and between any company named in any associated TRG Customer Order ("Customer") and Express Badging Services Inc. ("Supplier"), effective as of December 15, 2026 (“Change Order Effective Date”) with an expiration date of December 14, 2028 (“Change Order Expiration Date”) pursuant to Agreement #EXPRE-0278332 between Ascension Health Resource and Supply Management Group, LLC ("The Resource Group" or "TRG") and Supplier, effective December 15, 2025 ("Agreement"), the associated and approved Customer Order (“Customer Order”), associated previously issued Change Order No(s) ("Previous Change Orders"), and is subject to the terms and conditions of the Agreement, the Customer Order, and the Previous Change Orders unless otherwise noted herein.

2. CHANGES TO PRODUCTS
Note: Indicate whether the Products in the associated TRG Change Order replace the previous Products or are in addition. Include any invoicing milestones applicable to the Products. Indicate whether the Products are subject to acceptance testing and whether they will be delivered (and if so where they will be delivered). If applicable, indicate the applicable license term.
EBS Usage Note:
Product-related Change Orders are expected to be infrequent, as most Customers issue new Purchase Orders or submit additional TRG Customer Orders when purchasing additional Products.

3. CHANGES TO SERVICES
Note: Include any changes to the description of the Services or deliverables and the Project Schedule. Indicate whether the Services in the associate TRG Change Order replace the previous Services or are in addition. Include any invoicing milestones applicable to the Services.

🔹 EBS Usage Note:
Service-related Change Orders are the most common because changes in scope often occur after the Customer sees the initial functionality. These scope adjustments—frequently Customer-driven feature requests—can increase required labor hours and therefore must be documented and approved through a TRG Change Order.

4. REVISED MATERIALS & EXPENSES
Note: Items in an associated Change Order will be representative of the existing materials and expenses and any added hereunder. The revised maximum below should take into account the fees authorized under the Customer Order and Previous Change Orders.

5. REVISED NOT-TO-EXCEED AMOUNT
Under no circumstances shall the total amounts payable for Services (including fees, materials, and expenses) exceed the revised Not-to-Exceed Amount documented in a TRG Customer Order or updated value within a previous Change Order; unless expressly approved in writing by Customer or documented in an newly executed TRG Change Order.
